To request an entry form, simply fill in your details in the form on the right.
You will receive an email from me with the link to the editable pdf of the entry form.
Then all you have to do is think about which category/ies you wish to enter and fill in the entry form accordingly (fill one entry form per category) and email it back to me using the details below.
